Memorial Wall Plaques
If you'd like to honor your departed companion,
and help CHS to continue serving the community, please consider purchasing one or more plaques.
CHS MEMORIAL WALL PLAQUES
Instructions and ordering information:
Just PRINT an order form, and select from the
animal outline pictures available.
Please use a separate order form for each order. (Xerox copies are fine; Additional forms
are available on request at the CHS Office.)
Payment by check or in cash is due at time of order. (If ordering by mail, please pay by check.)
You can place your order in person at the shelter during normal business hours (see below).
Material: Plaques are engraved locally (in Scappoose), on a two-layer
material resulting in black letters on a gold background.
Sizes: Two sizes are available:
- 2½ x 7 inch: Up to 50 characters plus your pick of one picture
(see choices on reverse) or up to 75 characters without a picture
- 2 x 5 inch: Up to 40 characters plus your pick of one picture (see choices on reverse)
or up to 65 characters without a picture
Suggested format:
- Name of Pet
- Birth date (if known) – Date of Death
- Words of Remembrance
